Team‑based care (also known as the Patient’s Medical Home) brings family physicians together with nurses, mental health providers, pharmacists, and other health‑care professionals.
Working as a team improves access to care, strengthens continuity, and supports patients through every stage of life.
The Innovation Fund: Building Health Care Teams
The Saskatchewan Medical Association and the Government of Saskatchewan have established a $10 million Innovation Fund to support family‑physician‑led, team‑based care.
Innovation Fund projects are helping clinics add team members, improve processes, and build stronger, physician-led teams.
