Are you moving to another clinic with an existing EMR? Let us assist you with the transition.

To support you, your SMA Practice Advisor will need to know:

• The contact information of both your current clinic and the clinic you will be joining 
• The date of your last day at your current clinic and the date that you will start at the new clinic
• The EMR software that you are using at your current location and the EMR software you will be using at your new clinic (Accuro or Med Access) 
• A list of the integrated services (i.e., electronic labs, the integrated viewer) you are utilizing.
• Will you require a data transfer of your current patient files?
• Have you submitted a new EMR Program application for funding?
