If you are interested in requesting a Pandemic Physician Services Agreement, the process is outlined below:
Submit inquiry via the form on the SHA website. Once you submit, a confirmation screen will appear, you will not receive an email confirmation. If you haven't done so, please then proceed to complete the Skills Inventory Submission.
The SHA will review the submitted form and make best efforts to provide you with an individual agreement for signature in no more than 5 business days
SHA creates individualized agreement and sends the agreement by email the physician. The agreement and invoice template be will be emailed to you for completion and signature along with an outline of invoice periods.
Interested physicians may commence on the agreements as early as April 6, 2020, as long as you submit the form below prior to 11:59 p.m. on Friday April 17, 2020. If you signal your interest later, the agreement start date will be the date you submitted the online form. SHA staff will make best efforts to provide you with the agreement for signature in no more than five business days. The agreement will be emailed to you for signature.
The effective date of these contracts can be April 6, 2020, and run until June 30, 2020. The basic rate for Pandemic Physician Services Contracts will be as follows:
