1. Personal Information
The SMA does not automatically collect personal information, such as name, home address, home phone number, personal email address, IP address and other personally identifiable information, (“Personal Information”) from visitors of the SMA website. In some instances, the SMA will collect personal information that is voluntarily provided by visitors when they register for access to the “members” section of the SMA website or when members subscribe to or “opt in” to receiving electronic communications such as newsletters, facsimile and email updates and advisories pertaining to the SMA, CMA, the College of Medicine, Regional Health Authorities and the Ministry of Health or when they participate in online discussion forums through the member section. The SMA will collect and use such personal information solely for the purpose(s) disclosed by the SMA before or at the time of collection and only after the visitor or member has voluntarily agreed to such collection and use, by clicking “I agree” on the online form or in writing if registering via faxed or mailed form. All electronic communications from the SMA to its members include instructions on how to discontinue receiving such communications and, except in limited circumstances, members can discontinue such communication at any time. Email addresses and facsimile numbers from members who wish to discontinue receipt of the SMA’s electronic communications will be removed from the SMA’s distribution list and databases.
Personal information that has been collected by the SMA is not disclosed in any identifiable form to any party outside the SMA except for the fulfillment of the specific purposes identified at or before the time of collection. For example, the SMA might disclose limited information in a secure fashion to an independent company to determine the personal well-being of members or the effectiveness of current programs delivered to the SMA membership. At any time, a visitor or member may send an email to firstname.lastname@example.org to request that their personal information be changed, removed or updated in the SMA’s databases.
2. Anonymous Non-Personal Information
When visiting the SMA website, anonymous, aggregated, and non-personal information is automatically collected. Such information may include the length and date of the visit, how the visitor navigated the website, what pages the visitor viewed, the type of browser being used by the visitor, the type of operating system used by the visitor and the domain name of the visitor’s Internet service provider. The SMA uses this anonymous, aggregated and non-personal information to track the success of its website and to tailor the website to meet the needs and interests of its members. This information may be shared with an outside organization or collected on behalf of the SMA to provide these or similar services.
3. Cookie-Based Information
Protection of Members’ Personal Information: The SMA protects the Personal Information it collects with appropriate technological, physical and administrative safeguards to protect it from unauthorized disclosure or use. Access to personal information collected by the SMA is limited to authorized individuals and is stored on its databases, which are protected by firewalls and are password-secured. The SMA retains the personal information only for as long as is required for the purposes identified at the time of its collection and consented to by the visitor providing it or as otherwise required by law. Once personal information is no longer necessary for the purposes consented to by the visitor, it is the SMA’s practice to delete it from its databases or systems or make it anonymous.
Linked Websites: The SMA website may be linked to other websites. These linked websites are not under the control of the SMA and are required to have their own privacy policies. The SMA is not responsible for and shall not be held liable for any procedures, policies or activities of any websites linked to the website.
